 CWF1 - Joint Notification of Self-Employment

 CA5601 - Application to pay Class 2 National Insurance Contributions by direct debit.

 CA72A - Application for deferment of payment of Class 1 National Insurance Contributions (for an employee or office holder).

 CA72B - Application for deferment of payment of Class 2 and Class 4 National Insurance Contributions.
